· Provides personal hygiene services, housekeeping tasks and other related support services essential to the consumer’s health

· Observes, records and reports all changes to supervisor

• Performs home management tasks including housekeeping, laundry, shopping and errands
• Prepares and serves simple modified diets according to instruction and assists with feeding as necessary
• Assists with bathing, dressing and grooming
• Assists with toileting, including use of bedpan, commode or toilet
• Assists self-directing consumers with transfers and ambulation including use of cane, walker, and wheelchair
• Assists self-directing consumers with medication reminders or refilling the consumer’s prescription at the local pharmacy as specified on plan of care
• Provides routine skin care. May assist self-directing consumers in applying nonprescription topical medications to skin surface
• Collects routine specimens
• Obtains consumer’s weight
• Assist self-directing consumers in reminding them of their home maintenance exercise programs
• Assists the self-directing consumer with reminders on changing the tubing or refilling a tank in the use of their oxygen equipment

Qualifications:

· Must have a phone or a way to communicate to the office daily

· Must have a valid PA License

· Must be able to follow the care plan written in the home

· Must provide proof of

o TB Test

o Criminal Background Check and FBI when necessary

o PPD documentation within one year of hire

· Must prove competencies in the 16 areas required under 28 Pa. Code §611.5
