Job Description

Home care Hospice Registered Nurse We are looking for a Registered Nurse to provide care for people who are terminally ill. The ideal candidate will have a passion for providing quality care to patients in a home health setting and demonstrate excellent communication and organizational skills. The successful candidate will provide assessments, develop care plans, administer medications, and monitor patient progress. Responsibilities:

• Frequently visiting patients to monitor vital signs and physical comfort levels.
• Make sure the clients' medications are refilled.
• Informing physicians about changes in patients' conditions.
• Notifying attending physicians, hospice staff members, and family members of patients' deaths when they occur.
• Collaborating on terminally ill patients' care with physicians, social workers, chaplains, nursing assistants, and other stakeholders.
• Prepare and adjust patient care plans as needed.
