Community Health Nurse – Bronx, NY
Available Schedule Tuesday-Saturday shift Sunday-Thursday shift. Monday-Friday shift with 1 weekend every 6 weeks. The Community Health Nurse will be making 4-5 visits per day. Home care experience is required.What You’ll Do:
Conduct skilled nursing visits and health assessments
Deliver person-centered care using the full nursing process
Educate members and families on health management
Maintain documentation in accordance with regulations and policies
Collaborate with the care team to support holistic patient needs
Education:
NYS licensed RN Certifications: BLSWhat We’re Looking For:
NYS Registered Nurse license
Prior experience with community health, home care, or PACE preferred
Strong assessment, communication, and organizational skills
Commitment to culturally sensitive and compassionate care
