Job Details Level : Experienced Job Location : Winslow Indian Health Care Center - Winslow, AZ Position Type : Full-Time Education Level : Bachelors Degree Salary Range : Undisclosed Travel Percentage : Frequent Job Shift : Day Description Under the general supervision of the Director of Quality Management, the Sr. Community Health Nurse provides comprehensive home visit nursing services using a referral-based process. The incumbent collaborates with care coordination and case management teams to address the needs of referred patients and families, working with community partners to tackle social determinants of health impacting patient outcomes. Responsibilities include assessment, nursing diagnosis, counseling, and prevention services. The nurse functions autonomously and interacts routinely with other resources, facilitating access to services for patients. Essential Duties & Responsibilities: Maintain regular attendance and punctuality. Apply ethical principles and demonstrate leadership in public/community health nursing practice. Advocate for the health, safety, and rights of individuals, families, communities, and populations. Act independently within professional nursing standards, scope of practice, and established physician standing orders to identify and address health needs during home visits. Implement patient-centered home visit nursing care within care standards and scope of practice. Use critical-thinking strategies to evaluate community health issues. Determine appropriate actions such as referrals to clinics or EMS based on assessments. Coordinate psychosocial aspects of patient care. Collaborate in planning, implementing, and evaluating comprehensive care plans with the team, utilizing evidence-based practices. Provide education and case management for patients with chronic diseases via home visits. Offer self-management education to clients. Maintain standards of care for chronic illnesses and home visit policies. Facilitate community self-management education classes. Prioritize care for high-risk clients. Document patient encounters accurately in Electronic Health Records, including self-assessment and goal setting. Engage in primary, secondary, and tertiary prevention activities, e.g., health fairs. Consider safety, effectiveness, cost, and community impact in planning care. Assist in developing and revising program policies, procedures, and protocols. Participate in quality assurance activities to improve care access and outcomes. Maintain proficiency through continuous education and team participation. Complete tasks as assigned by supervisors. Uphold confidentiality and ethical standards. Adhere to organizational policies, rules, and regulations. Interact professionally and ethically with all stakeholders. Demonstrate cultural awareness and sensitivity. Comply with HR requirements. Perform other duties as assigned. Qualifications Minimum Qualifications: BSc Nursing or entry-level MSc Nursing, with at least 2 years' experience in nursing or public health. Must be licensed RN in AZ, CPR/BLS certified. Public health nursing certification and bilingual skills preferred. Valid driver’s license, background check, and compliance with Child Care requirements are mandatory. Knowledge, Skills, Abilities: Knowledge of public/community health nursing principles, theories, and core functions. Ability to incorporate Patient-Centered Medical Home principles. Knowledge of health education, anatomy, physiology, pathophysiology, epidemiology, pharmacology, and community health systems. Competence in documentation, emergency preparedness, and use of health technology. Ability to travel for home visits, demonstrate professional conduct, communicate effectively, and work collaboratively. Dependability, flexibility, cultural sensitivity, and ability to work under pressure. Physical Demands Regular walking, sitting, using hands, reaching, talking, hearing; frequent standing, climbing, stooping; occasional lifting up to 50 pounds. Work Environment Office or home settings with moderate noise; exposure to risks like bloodborne pathogens; may require evening/weekend hours and extended shifts.
